Policies & Forms

Before your first appointment, please review our clinic policies. The policies help keep care clear, respectful, and consistent for every client.

Informed Consent

  • Massage therapy is non-sexual in nature and intended to support wellness, stress reduction, and muscular relief. 

  • Massage therapy is not a substitute for medical diagnosis or treatment.

  • Massage therapist do no diagnose medical conditions, prescribe medications, or perform spinal adjustments.

  • Please inform your therapist of relevant medical conditions/medications and speak up immediately if you experience pain or discomfort.​

Cancellation, Rescheduling & No-Show Policy

Appointment time is reserved specifically for you. Cancellations may be made by voicemail, text, email, or speaking directly with staff. The time stamp of your message determines notice.​

  • 24 hours or more notice: No charge. Any prepayment may be credited to a future appointment or refunded upon request.

  • Less than 24 hours notice: 50% of the scheduled fee will be charged. The remaining 50% may be credited to your account for future use.

  • No-show or notice of 4 hours or less: 100% of the scheduled service fee will be charged.

  • Late arrival may result in a shortened session without a reduction in fee.

Exceptions may be considered for emergencies or contagious illness at management discretion.

Privacy & HIPAA

  • Your protected health information is kept confidential and will not be released without your written authorization, except as permitted or required by law.

  • If you self-pay, release of records to any third party requires a signed Release of Information (ROI). 

Authorization to Bill Insurance

  • You authorize Body Madix Massage Therapy LLC to submit claims, receive payment directly, and release records as required for claim processing. 

  • Insurance verification is a courtesy and does not guarantee payment. You are responsible for knowing your benefits, copays, deductibles, and authorization requirements. 

Financial Responsibility

  • You are financially responsible for services rendered, regardless of insurance coverage or reimbursement. 

  • Copays, coinsurance, and deductibles are due at the time of service unless prior arrangements are made. 

  • Self-pay clients are required to pay in full at the time of booking to reserve an appointment.

  • Any balance not paid by insurance remains the client's responsibility.
